Secondary gain is often unconscious and the patient is unaware that they are capitalising on the benefits of having a problem. If it is a benefit that the patient is aware of (and deliberately fabricating or creating in order to benefit) this is known as malingering. 

Secondary gain is very difficult (especially for those using NLP) to identify but if you have some strong suspicions here are some strategies to help deal with it.
